How to Report an Incident?

In case of an accident/incident or near miss you must submit the Incident Report.

3 easy steps :

  1. Download the form below.
  2. Complete the form. Ensure all the fields are filled in.
  3. Please sent the completed form to admin@mccareconnect.com.au with the subject line Incident Report.

Mc Care Connect is a platform to connect Support Seekers with Support Providers in a safe and easy environment. Mc Care Connect offers accessible support using Independent Support Providers. All Support Providers are required to  have Working with Children Check Certificate and undertake a NDIS Worker Screening Check.
